Preliminary investigations show there may have been “significant human error” by the captain of cruise liner that ran aground off Italy.

A statement from Costa Cruises, the company which owns the Costa Concordia, says their immediate priority is to account for all passengers and crew, and secure the vessel.

Five people are now known to have died in the disaster – more than a dozen are still missing.
