The HSA has been called in to investigate following the death of a toddler on a farm in West Cork.
It's believed the two year old from Schull was hit in the head by a steel gate around lunchtime today.
The child was airlifted to Cork University Hospital, but he was declared dead after efforts to save his life failed.
